Physician: 'I almost killed a patient' due to advance directive

According to the author Philip Betbeze: While advance medical directives can be a benefit to patients, families and health care costs, misinterpretation of these documents by clinicians is common, says a prominent emergency medicine physician. This can lead to irreversible medical errors. I recently wrote about the idea of making advance directives a condition of obtaining Medicare benefits. Such a move, one major healthcare CEO told me, would be a great way to help reduce the cost of care for patients at the end of life, where so much expensive treatment with dubious results takes place.

HHS updates ICD-10 requirements
ACMA
As a result of the enactment of the Protecting Access to Medicare Act of 2014, the Department of Health and Human Services (HHS) expects to release an interim final rule soon that requires the use of ICD-10 starting Oct. 1, 2015. All HIPAA-covered facilities will still be expected to use ICD-9-CM through Sept. 20, 2015.

Additionally, the ICD-10 end-to-end testing scheduled for July by the Centers for Medicare and Medicaid Services (CMS) has been canceled.

Save the date: 2014 Leadership Conference and Medical Director Forum

Mark your calendar and make plans to join us for the 2014 ACMA Leadership Conference and Medical Director forum in Orlando, Florida. These events will be held at the Orlando World Center Marriott, Nov. 19-22.

Leadership Conference: Nov. 19 and 20 The ACMA Leadership Conference is a two-day event designed specifically for health care delivery system case management and transitions of care professionals in leadership positions. A faculty comprised of practice leaders and health care experts will address pertinent issues impacting case management and care delivery.

Medical Director Forum: Nov. 22 Initially planned for April 10, in Chicago, this intensive workshop has been rescheduled to occur in conjunction with the 2014 ACMA Leadership Conference on Nov. 22 in Orlando.

During this full-day event, experienced physician advisors, medical directors and case management leaders will discuss timely topics impacting the unique role of physicians in the case management model.

CMS releases 2015 IPPS Proposed Rule
ACMA
The 2015 Medicare Inpatient Prospective Payment System (IPPS) Proposed Rule includes several decreases in Medicare payments to acute-care hospitals effective Oct. 1, 2014, if approved. A recent article published by Becker's Hospital Review outlines 10 important points regarding the proposed IPPS for FY 2015.

Included in the proposed IPPS is:
  • A 1.3 percent increase in Medicare operating rates for acute-care hospitals that report quality data and are also meaningful users of electronic health records
  • A 1.1 percent reduction in disproportionate share hospitals (DSH) payments for FY 2015
  • No substantive change to the two-midnight rule.

What is the 'Since You Asked Committee' and why is it important to you?

The "Since You Asked" Committee has convened for 2014-15 with the goal of drafting short articles that answer common issues faced by case managers. The articles have consisted of regulatory issues, how to have a crucial conversation, what is an LCD versus an NCD, MAC pre-payment review, and last but not least, the 13 items required for the conditions of participation for discharge planning for Medicare patients. Our purpose is to have you, the reader, pose a question to the committee. Then, the committee does the research to get the best information on the subject and writes an article on the findings. The articles are researched and the information is referenced to allow the reader to seek out more information from the research.

To get your question answered, submit suggestions to Nancy Loeffler, the committee chair, via email at Nancy.Loeffler@inova.org. She and the committee will then consider the request and how best to approach the subject.

Payment reform landscape: Payment for non-visit functions, medical home

According to the author Suzanne Delbanco: As I've been discussing in Health Affairs Blog each month, payment reforms can pose a spectrum of financial risk for providers, with financial upside only — such as pay-for-performance programs — on one end, and downside-only models — such as nonpayment for care that shouldn't happen — on the other. In March, we examined pay-for-performance, an-upside only model. Now we look at another upside-only model, typically used to support care coordination and patient centered medical homes.
