Readmission reduction push shows promising results
FierceHealthcare
Despite considerable criticism regarding the Hospital Readmissions Reduction Program, the initiative is largely doing the job that the Affordable Care Act created it to do, according to a new study published in Health Affairs. The Centers for Medicare and Medicaid Services hit more than 2,600 hospitals with financial penalties last year for their high readmission rates and increased the penalty ceiling from 1 percent to 3 percent of reimbursements, FierceHealthcare has reported.

Case Management Advocacy: A call to action
ACMA
Bills have been introduced in Washington, which address issues affecting your daily practice and adversely impacting patient care. The Improving Access to Medicare Coverage Act and the Medicare Audit Improvement Act represent opportunities for case managers to voice concerns related to observation status/the three-day stay requirement and the CMS Recovery Audit Program (formerly RAC). Over the coming months, you will periodically receive email calls to action regarding these issues, which contain a link to ACMA's online Grassroots Action Center. You can also learn more and make your voice heard anytime at www.acmaweb.org/action. Please take a few moments and contact Congress to request support of these bills. ACMA has already provided a message to help you get started — simply customize/add your personal experience, sign your name and send. One click, it's that easy. There is strength in numbers, so we also encourage you to share these issues and direct your colleagues and non-member advocates to join the effort via ACMA's Grassroots Action Center at www.acmaweb.org/action.

CMS releases final rule on MSSP ACOs
FierceHealthcare
The Centers for Medicare & Medicaid late Thursday released its final rule on the Medicare Shared Savings Program in an effort to encourage more participation of accountable care organizations (ACOs) in risk-based models. The final rule simplifies rules for ACOs and reduces administrative burdens but also takes into account concerns that industry experts had about the proposed rule, and made additional changes to ensure the program survives.

Why healthcare organizations must embrace the 'Quadruple Aim'
FierceHealthcare
The movement to add a fourth component to the healthcare industry's ideal of the Triple Aim appears to be gaining steam. In its current form, the Triple Aim — better care, better health and lower costs — fails to acknowledge workers' critical role in transforming healthcare, Rishi Sikka, of Advocate Health Care, Julianne Morath, of the Hospital Quality Institute, and Lucian Leape, of the Harvard School of Public Health, write in an opinion piece in the BMJ Quality & Safety journal.

American Muslim RNs reflect on challenges, opportunities to promote healing
Nurse.com
When some patients see Aisha Hasan-Rasool's long sleeves under her lab coat and her headscarf, they want to know where she's from. "I tell them Buffalo, New York," said Hasan-Rasool, RN, WHNP-BC. Hasan-Rasool is a nurse practitioner at a small women's health clinic in Duluth, Georgia. Like nearly half of all Muslims born in the U.S., she converted to Islam. If people want to know more about her faith or customs, she readily answers questions, but often it goes no further than the initial inquiry.

To adjust or not to adjust: Examining the role of risk adjustment in population health
FierceHealthcare
For 30 years, Medicare's hospital Inpatient Prospective Payment System has used Diagnosis Related Groups to account for the differences in the cost of care for clinically complex patients. By creating a direct link between reimbursement and outcomes, pay-for-performance adds a new layer to the issue of risk adjustment. Myriad quality metrics are currently used in healthcare — for comparing providers to each other, for informing consumers and clinical teams, and for establishing payment bonuses or penalties to incentivize improvement.

Beware of cloned medical records
Eisneramper
More health care providers utilize electronic health records than ever before. In part, this is due to government incentives. As the transition from paper medical records to EHR continues, payers and auditors are noticing a problem with these records: cloning. According to Medicare,"documentation is considered cloned when each entry in the medical record for a beneficiary is worded exactly like or similar to the previous entries. Cloning also occurs when medical documentation is exactly the same from beneficiary to beneficiary."

To improve patient experience, some hospitals redesign dining areas
FierceHealthcare
To maximize patient satisfaction, many hospitals cast an eye toward the design of their dining spaces, according to Healthcare Design Magazine. Although these areas are not major revenue drivers, they play a large part in satisfaction and patient experience, Yi Belanger, a designer at Add Inc., told the publication. An outdated or poor-quality dining area may create the impression that the overall facility suffers from the same problems. As a result many hospitals want to change the traditional single-serving-line model, which can create confusion between entry and exit points, according to Belanger.

Study: Best practices for the primary care practice
By Christina Thielst
The healthcare delivery system and payment models are changing. Much more emphasis is being placed on preventative care, engaging patients and their family caregivers as members of the care team, and improving satisfaction and outcomes. A recent analysis of commercial insurance data provides some hints at what primary care practices — as well as some specialty or multispecialty practices — can do to improve their performance and bottom line. The study found that those practices who consistently delivered exceptional value to their patients spent 58 percent less than those cared by their peers nationally.
