3 population health tips from a triple aim 'early adopter'
Institute for Healthcare Improvement
According to authors Leslie Graham and Barbara Eldridge: During our years working with IHI, we've learned how to do rapid testing and scale up at the local level. We've also consistently set aims, developed measures, collected data from the beginning of our projects, and used data to drive where we’re going with our projects and programs. It's safe to say we're very familiar with the most important basics IHI teaches about improvement. At the same time, we are keenly aware of the importance of being open to continuous learning. We hope our lessons learned will be useful to others on their population health journey.

Observation status, ED visits create illusion of fewer readmissions, quality gains
FierceHealthcare
Even as the government claims preventable hospital readmissions have dropped, a dramatic increase in hospitals' use of observation status and emergency care in lieu of readmission suggests many providers are gaming the quality standards, according to a Health Affairs blog post. Hospitals' use of observation status has been a source of controversy in recent years. Critics say the designation confuses patients; states such as Virginia have passed legislation requiring hospitals to inform patients when they are designated under observation status.

Data transparency: What the numbers don't tell us about quality healthcare
U.S. News & World Report
According to author Elaine Cox, M.D.: We live in an era where healthcare consumers desire data to make informed decisions about where they will seek care and from whom. We urge transparency in patient safety information and discuss the need for "evidence based medicine" incessantly. But what is full disclosure and is it healthy? Every type of company has truckloads of data, be it market data, polling data or public opinion data.

3 reasons ACOs will thrive
FierceHealthcare
Although the Medicare Shared Savings Program has many flaws, including setup and operating costs, and at least two more Pioneer accountable care organizations (ACOs) may drop out of the model, ACOs show great promise for improved healthcare delivery, according to Paul Keckley, Ph.D., managing director of the Navigant Center for Healthcare Research and Policy Analysis. The latest data from the Centers for Medicare & Medicaid Services show that although the program generated $411 million in total savings in 2014, three in four ACOs didn't save Medicare money, Keckley writes in a blog post for Pulse Weekly.
