CMS rolls out second phase of initiative to prevent nursing home resident hospitalizations

The Centers for Medicare & Medicaid Services has announced the second phase of its initiative to reduce preventable hospital admissions among nursing home residents. Avoidable admissions are a major problem among nursing home residents, accounting for up to 45 percent of admissions among beneficiaries of Medicare or Medicaid's nursing facility services, according to CMS. This works out to about 314,000 hospitalizations and $2.6 billion in Medicare funds a year.

'Behavioral economics' may improve medication adherence efforts

The newspaper examined ongoing public and private sector initiatives to get patients to adhere to their doctor's medication instructions. Patients who don't follow the advice of their providers cause a significant amount of waste in the industry, accounting for 13 percent of overall healthcare spending.

CMS announces Equity Plan, targets Medicare disparities for poor, seniors, minorities

The Centers for Medicare and Medicaid Services on Tuesday announced a plan to reduce disparities in healthcare for Medicare beneficiaries who belong to minority population, live with a disability or who reside in a rural area. The Equity Plan for Medicare focuses on six key areas over the next four years: the collection and analysis of data; the integration of solutions across CMS programs; the dissemination of approaches to reduce health disparities; increasing the ability of healthcare workers to meet the needs of vulnerable populations; improving language access for those with limited English proficiency; and increasing physical accessibility to health facilities.

Rush University Medical Center joins online review-posting trend

Rush University Medical Center has become the first organization in the Midwest to post the results of patient surveys in the profiles of individual physicians on its website. The organization's announcement comes just a few weeks after FierceHealthcare reported that Long Island-based North Shore-LIJ Medical Group also plans to post patient reviews of its doctors online, making it the first large medical group in the New York metropolitan area to do so.

California passes bill delaying transfer of fragile kids into managed care

California legislators passed a bill postponing a controversial plan that would have shifted tens of thousands of medically fragile children into Medi-Cal managed care plans. The bill, AB 187, passed both houses Thursday and will be sent to Gov. Jerry Brown for his signature. At issue was the fate of the California Children's Services program, which serves an estimated 180,000 children younger than 21 with serious medical conditions, including spina bifida, cancer, cystic fibrosis and sickle cell disease.

How a California health system's holistic approach prevents unnecessary admissions

Heritage Medical System's "holistic" approach to care is "not driven by a hospital" but by what's "right for the patient," according to the chief innovation officer at the Northridge, California-based health system. Taking care care of a patient "holistically" means the patient is at the center of his or her care, James May said in an interview with Hospitals & Health Networks, noting this involves a primary care network and appropriate specialties to take care of that patient. This approach, he said, significantly reduces unnecessary hospitalizations, at which point care is provided in ambulatory settings.

Forthcoming AHRQ funding to focus on patient safety

A new round of project funding from the Agency for Healthcare Research and Quality will focus on providing strategies and tools for improving patient safety in ambulatory care and long-term care facilities. The announcement doesn't stipulate an amount of money up for grabs, but says grants will not exceed $500,000 in any given year, and will not exceed $1.5 million for the entire project period. Projects may not exceed three years.

Sepsis prevention: Ohio collaborative develops protocols to attack deadly blood infection

Every day 38 Ohioans die from sepsis, the sixth most common reason for hospitalizations in the state. The blood infection contributes to half of all hospital deaths in the United States, including 13,000 people in Ohio each year. But a new joint initiative by the Ohio Hospital Association's Institute for Health Innovation and the Sepsis Alliance aims to attack the deadly blood infection and develop protocols that state hospitals can follow for early detection and intervention.
