Hospitals seek behavioral partners to reduce Medicare readmissions

For behavioral health organizations, the emerging movement among acute care hospitals to reduce readmissions represents both a challenge and an opportunity. The opportunity comes from the growing recognition that behavioral issues contribute to many of these hospital readmissions. If behavioral healthcare organizations can find ways to collaborate with hospitals, they can open a new avenue to grow their businesses.

Half of readmissions disparities are beyond hospital control

Health experts have long warned that federal readmission penalties punish hospitals for outcomes beyond their control, and now new research published in JAMA Internal Medicine suggests the patient population's clinical and social makeup accounts for half the difference between the lowest and highest performers. Under the standard Medicare formula, which only controls for patient age, sex and certain conditions, the top hospitals have 4.4 percent fewer readmissions than the worst performers.

One of the main ways the Affordable Care Act seeks to reduce health care costs is by encouraging doctors, hospitals and other health care providers to form networks that coordinate patient care and become eligible for bonuses when they deliver that care more efficiently. The law takes a carrot-and-stick approach by encouraging the formation of accountable care organizations in the Medicare program. Providers make more if they keep their patients healthy.

Hospital group: CMS star ratings lack vital socioeconomic, demographic factors

Medicare's five-star scale that rates the quality of care provided at hospitals doesn't offer a complete picture because it fails to reflect the distinct socioeconomic and demographic factors of vulnerable patients, according to hospital advocacy groups. America's Essential Hospitals and the Association of American Medical Colleges have urged the Centers for Medicare & Medicaid Services in recent weeks to add the income-related information into its ranking calculations.

Under pressure, hospitals push physicians to improve their bedside manners

A doctor's training hasn't historically focused on sensitivity. And too often while juggling heavy workloads and high stress, they can be viewed as brusque, condescending or inconsiderate. A 2011 study, for instance, found barely more than half of recently hospitalized patients said they experienced compassion when getting health care, despite widespread agreement among doctors and patients that kindness is valuable and important.

Medicare yet to save money through heralded medical payment model

A high-profile Medicare experiment pushing doctors and hospitals to join together to operate more efficiently has yet to save the government money, with nearly half of the groups costing more than the government estimated their patients would normally cost, federal records show. The Centers for Medicare & Medicaid Services offers the lure of bonuses to health care practitioners who band together as accountable care organizations, or ACOs, to take care of patients.
