Report: Provider teamwork key in reducing costly medical errors

Almost every American will experience a medical diagnostic error, but the problem has taken a back seat to other patient safety concerns, an influential panel said in a report out today calling for widespread changes. Diagnostic errors — defined as inaccurate or delayed diagnoses — account for an estimated 10 percent of patient deaths, hundreds of thousands of adverse events in hospitals each year and are a leading cause of paid medical malpractice claims, a blue ribbon panel of the Institute of Medicine (IOM) said in its report.

As the reforms of the Affordable Care Act continue to take hold, the so-called 'donut hole' coverage gap for the Medicare Part D drug discount program has been steadily closing as federal incentive payments give seniors a financial break when they've hit their drug spending limits. According to new data released by the Centers for Medicare and Medicaid Services on Monday, the U.S. government has paid nearly $2.3 billion through July in incentives to close the coverage gap in 2015.

Being responsive conveys respect, says one of the country's best-known hospital CEOs, right down to how quickly leaders respond to emails. Emailing directly with physicians, staff and even volunteers for the large San Diego-based Scripps Health healthcare system also gives chief executive Chris Van Gorder "information I could never get in a report or probably even in a group discussion," he told Becker's Hospital Review. Replying personally — and quickly — to those emails is part of adhering to the Scripps Health's core value of responsiveness, he says.

Burwell: For Obamacare, upcoming enrollment season will be toughest yet

HHS Secretary Sylvia Burwell says the administration estimates that nearly 18 million people have gained insurance so far as a result of the health law, but officials will now be setting their sights on the less eager consumers. Top Obama administration officials said they were anticipating their toughest sign-up season yet for insurance coverage under the health law. Officials aim to make a dent in the number Americans still uninsured in the law's third enrollment period.

VA scandal: Senate panel blasts continued whistleblower reprisals

The movement for reforms within the Department of Veterans Affairs has largely focused on reducing wait times at VA facilities, but advocates say a hospital culture in which whistleblowers are discouraged or punished persists. Now a whistleblower advocacy group says the department has made little progress on the issue a year later, according to the Associated Press.
