Reduction of inpatient care is challenging hospital revenues

While patients may be very pleased with the ability to stay out of the hospital, those healthcare organizations that are failing to adapt to a greater emphasis on preventative, outpatient care and shorter lengths of stays are finding empty beds to be a significant hit to their bank accounts. Small hospitals around the country are shutting their doors after becoming mired in debt and declaring bankruptcy, squeezed out by competition that is intended to save money for the healthcare industry at large.

The financing of healthcare is moving away from volume and toward value. Providers of all stripes will increasingly be compensated not on how much they do, but rather on how well they do it, and what outcomes they produce. Among other things, this transformational model of care is driving the huge spate of physician acquisitions by health systems. For the latter, managing cost — still largely driven by physicians — will be the hallmark of success, and industry performance will be more closely scrutinized than ever before.

Earlier this month, President Barack Obama signed legislation that delays the transition to ICD-10 coding by at least one year. In the wake of that announcement, providers, health insurers and others who were preparing to make the switch this October have been re-evaluating their implementation plans and awaiting further guidance from CMS (which is coming "soon," according to the agency's website). Part of their recalibration involves physician training initiatives.

Reactions to the ICD-10 delay, which has pushed off the new code set until at least Oct. 1, 2015, have been all over the map. Disappointed coders, ambivalent physicians and opportunity seekers on the financial side have all weighed in on the impact of the infamous provision of HR 4302. As the industry slowly moves past the disbelief and vitriol, however, the reality remains: ICD-10 has been stalled again, and the next 18 months will be a critical time for vendors and their clients to prepare for more than just the intricately detailed code set.

Physician practices shift tactics to get patients to pay medical bills

Between 2008 and 2012, multispecialty practices saw their bad debt go up 14 percent, according to a survey by the Medical Group Management Association, a trade organization for doctor practices. That's money that practices were owed but couldn’t collect. Some of them have begun to change their billing strategies to combat those debts, says Ken Hertz, a principal consultant with the MGMA Health Care Consulting Group.

Early evidence from Medicare's bundled payment pilots show improved quality at reduced cost of care

Early evidence indicates that Medicare's bundled-payment pilot has helped participating providers improve the quality of care while better managing healthcare costs. Should more detailed findings confirm these outcomes, Medicare could decide to expand the range of clinical services it wants covered by a bundled-payment arrangement. The model provides one payment for all services rendered by multiple providers for one episode of care. It is designed to be a test of this payment model for inpatient hospital acute care and post-acute care only. If successful, some experts predict the bundled-care model will likely be expanded to outpatient services too.
