Weighing an RCM upgrade

As health systems and hospitals straddle the two worlds of volume and value, it's tough to know what to do about upgrading revenue cycle management systems. Upgrade? Or get by on what's already there? Wake Forest University Baptist Medical Center's RCM system is out-of-date and is in the process of being replaced, but not before the medical center squeezes more life out of it with the assistance of programmers, who have installed upgrades that have helped to keep things up and running, said Ken Kubisty, associate vice president of corporate revenue for the Greensboro/Winston-Salem, North Carolina-area facility.

Revenue cycle poised for big rethinking

Since 2013, there's been "no change" – at least not anything appreciable – in the U.S. revenue cycle management market, says HIMSS Analytics Executive Vice President John Hoyt. Still, the fact remains that "the average system out there is quite old – nine years." That said, shifting imperatives for healthcare finance mean big changes are necessarily in the offing. It may just take a few years for other priorities to get squared away.

Value-based reimbursement coming, but not without challenges

The value-based reimbursement model predicted by many to dominate the reimbursement market is also the one they say will be hardest to implement. A new survey examines the state of value-based reimbursement, and found that of the existing value-based models, payers and hospitals/health systems predict pay-for-performance will experience the most growth – increasing from 10 percent today to 18 percent in five years for payers and from 9 percent to 21 percent for hospitals/health systems.

One element of preparation that should be on each provider's to-do list is dual coding and clinical documentation improvement (CDI). The process and scope of any dual coding and CDI effort will differ based on organizational goals. Key factors to consider when planning your dual coding and CDI include staffing availability, budgets, training schedules, and technology improvement initiatives. Healthcare providers want patients to read medical records, spot errors

Healthcare providers are giving patients more access to their medical records so they can help spot and correct errors and omissions. Studies show errors can occur on as many as 95 percent of the medication lists found in patient medical records. Errors include outdated data and omissions that many patients could readily identify, including prescription drugs that are no longer taken and incorrect data about frequency or dosage. Technology is giving the efforts a boost: More than half of doctors use electronic medical records, compared with just 17 percent in 2008, according to the federal government, which offers financial incentives to providers tied in part to giving patients access to their health information online.

CMS extends funding for low-volume hospitals

The Centers for Medicare & Medicaid Services extended a program to boost payments to hospitals with low Medicare inpatient volumes, according to a notice in the Federal Register. The extension of the program, which adjusts for hospitals that participate in Medicare's inpatient prospective payment system but have a low volume of patients, applies to the second half of the 2014 fiscal year and will now run through Sept. 30.
