Hospital finance team and nursing leaders drive value jointly

Healthcare finance professionals must better understand how value is delivered by frontline providers, and simultaneously offer providers business insight, according to Pamela Thompson, CEO of the Association of Nurse Executives. Developing reimbursement models to match the care that is delivered is the critical next step for U.S. healthcare. "The value of nursing is beyond the individual task performed," said Thompson. "The value is the health outcomes achieved per dollar of cost spent."

Today's referrals to multiple specialists mean a slew of specialist notes must be created and shared. The number of tests and therapies now available also contributes to the wash of clinical data. As patients become more involved in their own healthcare decisions, requests for second and third opinions are becoming increasingly common. Add in family queries for records, the detailed data required by payers, and information mandated by meaningful use and other legislation, and it's easy to see why hospitals are bending under the weight of information overload.

ICD-10 implementation not as costly as feared

The cost for physician practices to implement ICD-10 medical diagnosis and in-patient procedure codes may not be anywhere near as high as feared, according to recent data from the American Association of Professional Coders. According to AAPC's Rhonda Buckholtz, vice president of education and training, a client analysis has found that "ICD-10 ready" medical practices with fewer than 10 providers have spent approximately $750 per person to prepare for the mandate," as reported in a podcast. "And the cost is even less for practices with 10 to 49 providers. To put this into context, AAPC has trained over 90,000 people toward some piece of ICD-10 implementation, making this cost information all the more compelling."

Study: Advanced EHR use brings 9.6 percent cut in patient costs

Use of an advanced electronic health record system can produce significantly lower per-patient costs in hospitals, says a recent study published in the American Journal of Managed Care, with healthcare organizations seeing nearly ten percent in savings from each hospital admission. While EHR adoption and the EHR Incentive Programs have been billed as a way to save billions of dollars for the healthcare system, concrete evidence of a true return on investment has been lacking, leading some providers to question the purpose of the massive and complex implementation process.

As the Patient Protection and Affordable Care Act's provisions are implemented, quality metrics will become a bigger agenda item in a hospital's board room. Medicare's quality incentive program has sent a large signal to other insurers and the healthcare industry at large with its risk-based contracts to achieve quality and cost targets via incentives, or in some cases, financial penalties. Additionally, both payers and purchasers have stepped up their demand for high-value healthcare with the start of mandated insurance changes this year. Cash-only looks good to doctors

One small, but growing, trend in healthcare is the movement of providers dropping from the roles of managed care and taking only cash-paying patients. Experts say that cash-only practices are a workable business model when done the right way, but it may be disruptive to others in the market. The number of physicians making the change is hard to pin down, but a 2008 report from the Center for Studying Health System Change approximated that 12.4 percent of physicians no longer accept insurance in the practice.
