Dealing with insurers costly for US practices

Medical practices in the U.S. spend nearly four times as many hours a week dealing with insurers than do practices in Canada, at nearly four times the cost, researchers found. U.S. doctors spend only about one hour more each week interacting personally with health plans than their neighbors to the north, but practice staffers in the U.S. spend nearly 10 times more hours dealing with insurance companies than do the staff of Canadian practices, according to a report. Study casts doubt on effectiveness of hospitalist care

Compared with those treated by their primary care physicians, hospitalist patients are more likely to visit an ED and be readmitted. The proportion of inpatient care provided by hospitalists has more than quadrupled since 1995, and many studies have found that patients cared for by hospitalists go home sooner than those who receive hospital care from their primary care physicians. Shorter lengths of stay save hospitals money and could be a sign of higher-quality care, but a new study sheds light on how hospitalist-treated patients fare after they are discharged. More

At present, providers are paid on an à la carte basis — each test, each hospital admission is paid individually. The more tests, the more hospitalizations, the higher the pay for providers.
