    307,000 veterans may have died awaiting Veterans Affairs health care, report says

    Hundreds of thousands of veterans listed in the Department of Veterans Affairs enrollment system died before their applications for care were processed, according to a new report. The VA's inspector general found that out of about 800,000 records stalled in the agency's system for managing health care enrollment, there were more than 307,000 records that belonged to veterans who had died months or years in the past. The inspector general said due to limitations in the system's data, the number of records did not necessarily represent veterans actively seeking enrollment in VA health care.

    Misdiagnosis: National movement afoot to address diagnostic errors

    Unfortunately, diagnostic errors are not rare cases. The statistics are alarming. Last year a study in BMJ Quality & Safety revealed that doctors make 12 million outpatient diagnostic errors each year--roughly one in every 20 diagnosis. But the rate may be even higher, according to the head of a new coalition formed to address dignostic errors. He places the rate at roughly 10 percent of all diagnostic encounters.

    Advocate blasts feds for keeping Medicare, Medicaid from physician-owned hospitals

    Physician Hospitals of America is calling upon Congress to change current federal law to allow new physician-owned hospitals to accept Medicare and Medicaid patients, according to the organization's president. "It was put in the bill so the big hospital lobby would support the ACA," said Blake Curd, MD, president of the Physician Hospitals of America.

    5 risk factors for unplanned readmissions identified

    A study published by the Journal of Evaluation in Clinical Practice has identified five factors that indicate high risk for unplanned hospital readmissions. The study, Risk Factors For Unplanned Hospital Re-admissions: A Secondary Data Analysis of Hospital Discharge Summaries, found an overall unplanned readmission rate of 5.2 percent in its analysis of the Be lgian Hospital Discharge Dataset of patients discharged in 2008. The study authors conclude hospitals need to target patients with five specific factors to forestall unplanned readmissions.
