Georgia dental board votes against proposed rules

Recent proposed rules required that a dentist will have to diagnose a patient before a
dental hygienist can do procedures like clean teeth and apply sealants. But those rules were voted down unanimously at a recent Georgia Board of Dentistry meeting. Elizabeth Appley, a lawyer who represents the Georgia Dental Hygienist Association, was at the meeting. She says there were more than 30 letters and testimonies against the rules.

General dentists, specialists often misdiagnose oral lesions

General practitioners correctly diagnosed only about half of the oral lesions they submitted for biopsy, while oral and maxillofacial surgeons and other specialists did slightly better, according to researchers from the Virginia Commonwealth University School of Dentistry. But the overall error rates across all groups were high enough to warrant the need for all excised
lesions to be microscopically analyzed for histologic diagnosis, the study authors noted. "If they do not feel comfortable doing a biopsy in a particular case, it should then be referred to an oral and maxillofacial surgeon," he said. "The important lesson is that any tissue removed by the general dentist should be sent for microscopic examination."

Dr. Stan Harn glances over his spread of primitive dental tools and grabs a T-shaped metal instrument with a hook on one end. This, he explains, is a turnkey. It looks like, and may have inspired, the modern-day basin wrench. The metal hook goes into a patient's mouth and latches onto a damaged tooth. The dentist clutches the handle and twists. Turn too hard and patients could — and sometimes did — lose a piece of jaw.
"You can see the amount of torque it generates," Harn said. "It was brutal." The turnkey is arguably the most cringe-inducing tool in a University of Nebraska Medical Center collection that includes foot-powered drills, wartime dental chairs and X-ray machines with exposed wiring.
