During transitions in care, unintentional medication discrepancies commonly occur and can threaten patient safety. To improve medication reconciliation, and in response to Joint Commission requirements, most hospitals have developed medication reconciliation processes. The successes of such programs have been varied, but there is now collective experience about effective approaches to medication reconciliation.

Sunday is the final day to apply for the ACM certification
Your next opportunity to sign up for the ACM certification ends Feb. 15, 2015 — applications and payment must be received by the ACMA National Office by this date.

The Health Care Standard
According to the 2013 National Hospital Case Management Survey, nearly half (47 percent) of case management departments prefer the ACM certification — the preferences of the remaining 53 percent of departments are divided among 4 certifications, with the ACM leading the next most preferred certification by more than 20 percent.

March 31 is the end of the annual open enrollment period when people who don't have coverage through their employers can sign up on or off their state's marketplace. With limited exceptions, people who miss this enrollment window will be unable to sign up for health insurance until next fall for coverage that starts in January 2015. In addition to being uninsured, you will face a penalty for not having coverage. The fine may be bigger than you expect.

Study: Hospital location is big factor in readmissions

Hospital performance accounted for 42 percent of readmission rates, while characteristics of the county in which the facility was located accounted for 47.5 percent, researchers reported in the journal Health Services Research. Issues related to access to care were strongly linked to readmission rates and the quantity and quality of nursing homes in the area also were factors, data showed.

HHS: 7.5 million enrolled through
AHA News Now
Nearly 7.5 million consumers selected or were re-enrolled in a health plan through between Nov. 15, 2014, and Jan. 30, according to preliminary data released by the Department of Health and Human Services. Almost 180,000 people signed up recently. The update includes data for 37 states using the platform and for select metropolitan areas.

The National Association of ACOs calls for improvements to Medicare ACO program

The National Association of Accountable Care Organizations has teamed up with physicians, hospitals, medical associations and almost all Medicare Shared Savings Program ACOs in the country to pen a 36-page letter in response to CMS and the Office of Management and Budget's December notice of proposed rulemaking to improve the MSSP. The letter calls on CMS to focus improving on the one-sided shared savings model.

Affordable Care Act startups in poor financial health

The Affordable Care Act's startup health insurance plans are flirting with financial distress, as all but five of the 23 nonprofit companies had negative cash flow from operations in the first three quarters of 2014, Standard & Poor's said in a recent report. The startups, called Consumer Operated and Oriented Plans, or co-ops, were created with $3.4 billion in federal loans as a counterweight to established health insurers. One of the largest of the companies, Iowa-based CoOportunity, has failed already and is being liquidated by the state.

Study: Readmissions after surgery associated with new postdischarge complications
In a study that included readmission information from nearly 350 hospitals, readmissions the first 30 days after surgery were associated with new postdischarge complications related to the surgical procedure and not a worsening of any medical conditions the patient already had while hospitalized for surgery, according to a study in a recent issue of JAMA. Readmission as a quality and cost-containment metric is now a major issue for hospitals, clinicians and policy makers. Financial penalties for readmission have been expanded beyond medical conditions to include surgical procedures.

Forget about repealing Affordable Care Act

If at first you don't succeed, try, try again? That certainly seems to be the motto of House Republicans. Recently, the House GOP took its 56th vote to repeal the Affordable Care Act, President Barack Obama's signature health care reform law. The bill's prospects for consideration in the Senate are low and the president has repeatedly promised to veto such a measure anyway. After 56 tries, the House votes to repeal the healthcare law have become so commonplace that hardly anyone in Washington even blinks an eye at them anymore.
