The 2014 MPFS and HOPPS: Projected impact on radiology

The proposed 2014 Medicare Physician Fee Schedule and Hospital Outpatient Prospective Payment System rules have both been released, and the outlook for radiology is dim, according to Cynthia Moran, assistant executive director for government relations and economics at the ACR. "The only good news — if you can call it that — is that CMS no longer appears to be targeting only radiology," Moran says.

Should fee-for-service reimbursement be eliminated?

Most physicians don't support a shift away from the fee-for-service model. They aren't fans of bundled payments, and don't want to take steps to add incentives in the system. That's according to a recent Journal of the American Medical Association survey. This is the same survey that found that physicians think others in the industry — trial lawyers, insurance companies, hospitals, device manufacturers and patients — bear a greater burden for lowering healthcare costs.

Physicians' Medicare pay data may be next to be made public

CMS is asking for public input on whether physicians have a privacy interest with their Medicare payments. If they do, CMS says it wants to create a review system that balances a physician's privacy with the public interest. Now that Medicare payment data for individual physicians is no longer sealed from public view, the Centers for Medicare & Medicaid Services wants help building appropriate policies that balance transparency and access to the data with privacy protections.

In states that don't expand Medicaid, some uninsured may still get help

Some of the millions of poor people expected to lose out on Obamacare coverage next year because their states are not expanding Medicaid might have a way to get help, but the strategy carries risk. Experts say the key is for them to project their 2014 income to at least the federal poverty level, about $11,500 per person or $23,500 for a family of four. That would entitle them to federal subsidies that would cover nearly all the cost of private coverage sold on new online insurance marketplaces set up by the federal health law.
