Among patients with infrapopliteal lesions, a self-expanding nitinol stent was associated with similar clinical outcomes at one year when compared with percutaneous transluminal angioplasty with bailout stenting, according to newly published results from the EXPAND trial. "The EXPAND study was the first multicenter randomized trial comparing [bare-metal stenting] using a new-generation self-expanding nitinol stent versus [percutaneous transluminal angioplasty] with bailout stenting for infrapopliteal arteries," the researchers wrote.

Millions of Americans take baby aspirin every day to prevent a heart attack or stroke. If they are at high risk of heart disease, they're doing the right thing, according to draft recommendations issued by the U.S. Preventive Services Task Force. The independent panel also said that taking low-dose aspirin daily for at least 10 years may protect against colorectal cancer, at least in people who are already taking it to prevent heart attacks and stroke.

Patients with an acute anterior ST-segment elevation MI who were injected with cyclosporine immediately before undergoing PCI did not have better clinical outcomes compared with a group receiving placebo, according to a randomized, double-blind trial. One year after surgery, 59 percent of patients in the cyclosporine group and 58.1 percent of patients in the placebo group had the primary outcome of death from any cause, worsening of heart failure during the initial hospitalization, rehospitalization for heart failure or adverse left ventricular remodeling.

Twenty-four months of dual antiplatelet therapy was associated with less stent thrombosis in patients with lumen narrowing but not in patients without lumen narrowing, according to recent findings. The researchers evaluated 1,754 all-comer patients to determine whether the presence of at least 30 percent luminal narrowing in the left main or proximal left anterior descending coronary arteries could influence outcomes with regard to duration of DAPT.

Suboptimal final angiographic flow grade is the primary driver of restenosis after subintimal tracking and reentry for chronic total occlusion recanalization, according to a retrospective study published online Aug. 26 ahead of print in Catheterization and Cardiovascular Interventions. The elevated restenosis rate associated with the STAR strategy makes it a second-line strategy, but as many new techniques rely on the same principle, "we believe that it is important to understand the reason" behind the worse outcomes, say Mauro Carlino, MD, of the San Raffaele Scientific Institute, and colleagues.

Cardiology providers at an urban tertiary care hospital used an Internet-based cardiac catheterization laboratory inpatient forecast tool to help predict daily demands for beds at the hospital. The automated tool extracts data from the catheterization scheduling system and forecasts daily cardiology bed needs. During the study period, the daily aggregate forecasts were accurate to within one bed for 70.3 percent of days and within three beds for 97.5 percent of days.

Bhatt, MD, MPH, chief medical editor of Cardiology Today's Intervention, discusses data presented at the European Society of Cardiology Congress on the use of protracted dual antiplatelet therapy in patients following an MI. A team of researchers performed a meta-analysis comparing longer and shorter durations of dual antiplatelet therapy in this patient population, and found that longer treatment was associated with reduced incidence of major adverse cardiac events, with increased risk for major bleeding, but not fatal or intracranial bleeding.
