Ethical dilemmas in dental assisting

Dental assistants are faced with choices in the course of performing their jobs every day. Will that impression require a maxillary or mandibular tray? Will the doctor need the 150 or the 151 forceps? Does the situation call for 21 mm reamers or 31 mm reamers? Many of these choices are cut-and-dried and based upon a definitive need in providing a dental service to a patient. However, whether we realize it or not, many decisions that confront dental assistants are truly ethical dilemmas. The choices we make in response to these dilemmas serve to determine the moral character of each dental assistant.

Periodontitis affects approximately 47 percent of adults in the United States and the U.S. Surgeon General has concluded that cigarette smoking is one of its causes. Despite numerous adverse health effects of environmental tobacco smoke, an association between periodontitis and this "second-hand smoke" in non-smokers remains questionable. In this study, the researchers aimed to investigate the association between serum cotinine and periodontitis among U.S. non-smokers.

QUESTION: Our office has a dilemma. The dentist we work for is doing inferior work. The hygienists are reluctant to recommend any work to patients knowing the quality will be poor. The patients are not happy and some have left. Those that stay and have work done have poor quality fillings, ill-fitting crowns, and are always coming back because a filling fell out or broke. This happens sometimes even within hours or days of having the work done. We have addressed these issues with him numerous times and have stated our concerns, but it doesn't seem to change anything.
