To keep patients healthy and safe during the pandemic, payers and the Federal Government temporarily relaxed restrictions, expanded the eligible provider list for providing telehealth services, and developed a reimbursement structure. Physical therapists were uniquely challenged during the pandemic, requiring a reassessment of techniques and strategies in order to best support patients. The purpose of this case report is to describe a musculoskeletal institution’s experience with implementing telehealth physical therapy.

In a survey we sent to physical therapy clinics in March 2021, 72.9% of respondents said they were seeing their patient volume increase.
That’s great, but here’s the hard truth: temporary relief does not mean recovery. What you are seeing now is a backlog of patients. If you want the same volume of patients coming to your practice in 6 months, you need to plan and control your recovery.

Physical therapy no-shows: Wasted opportunities

In order to optimize musculoskeletal care, a team of researchers acted on their curiosity about the frequency and reasons for no-shows at physical therapy (PT) appointments. Their retrospective cohort study, “Prevalence and predictors of no-shows to physical therapy for musculoskeletal conditions,” appears in the May 28, 2021, edition of PLOS ONE.
Nrupen Bhavsar, Ph.D., an assistant professor in medicine at the Duke University School of Medicine in Durham, North Carolina, and co-author explained to OTW how novel this particular study is. “To our knowledge, this is the first study to quantify the national prevalence of no-shows in the United States for patients seeking care for physical therapy.”

Telehealth speeds access to physical therapy

The VA Phoenix Medical Center launched a new telehealth physical therapy (PT) clinic as part of Veterans’ post-op care plans after total knee replacements. The innovation empowers Veterans to begin VA outpatient PT at home within a week of surgery, improving Veterans’ experience of care.
“Timely access to physical therapy is so important to get Veterans moving in a healthy pattern because sometimes, after surgery, we’re hesitant to move or we’re nervous,” said Christina Crawford, telehealth coordinator (pictured above). “Also, it can help to prevent tightness (known as contracture) in the knee, therefore improving the mobility of the knee.”
